Good Faith Estimate Notice
Effective Date: 6/1/2025
Under the No Surprises Act, healthcare providers are required to provide clients who do not have insurance or who choose not to use insurance with a Good Faith Estimate of expected charges for healthcare services.
Your Right to a Good Faith Estimate
You have the right to receive a Good Faith Estimate explaining how much your medical and mental health care may cost.
This applies if you:
do not have health insurance
choose not to use your insurance benefits
are paying privately for counseling services
The estimate includes expected costs for reasonably anticipated services related to your care.
When You Should Receive It
You have the right to receive a Good Faith Estimate:
when you request one
before your first scheduled service
before additional services are provided when appropriate
You may also request a Good Faith Estimate at any time before scheduling services.
If Your Bill Is Higher Than Expected
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill.
You should save a copy or photo of your Good Faith Estimate for your records.
